The goal of the Care Coordination program is to come alongside you, building on your strengths, and developing a solution-oriented care plan that will enhance your life. We have a vast library of community resources to draw upon. Our mission is to help you set and meet your personal goals. We can collaborate with you to enhance your safety and independence.
Following an assessment of your needs, we can provide supportive services, such as: Meals on Wheels, Home Care, Senior Safety Team for minor home safety modifications or other items, Friendly Visitor, Volunteer Shopper, Health Insurance Benefits Assistance, Respite for Caregivers, Seasons Adult Day Health Services, Transportation options, and more.
A Care Coordinator can provide referrals to Counseling services, Home Repair Programs, Legal Consultations, Waiver and PACE programs, Assisted Living locators, Veteran’s Services, Memory Support, and more.
