Our Care Coordination team is ready to assist when an individual is in need of personal guidance, resources, and services to maintain independence. Our programs and services enhance the quality of life for older adults throughout Midland County. It all starts with an in-depth assessment in your home to identify health, social, environmental, financial, and emotional needs.

Home visits allow for understanding of your situation and your goals. Home visits provide understanding of unique barriers that may be standing in the way of meeting your goals.
“Why do I need to let you into my home? I just want a shower chair.”
Home visits provide opportunities to identify unmet needs such as updated smoke detectors, stair railings, and grab bars, as well as other ways we can increase safety and security for you.
“Why do you need my financial information? My personal business is none of your business.”
During a home visit, we will ask many questions. It is your prerogative to provide answers or decline. By understanding your financial situation, we can provide resources such as a Medicare Plan Comparison to determine that you are getting coverage for the lowest cost. We can offer support groups and caregiver respite. We can connect you with community resources that you may not know about. We can assist with applications to home heating credit, Medicaid programs, Extra Help for prescription Part D drug costs, kinship assistance if you are caring for a minor child, and many others.
We take our responsibility to the older adults in Midland County seriously. We are equally diligent in meeting the requirements for access to services and programs, which are made possible through millage and state grant funding. Following the in-home assessment, we offer the services for which you are eligible. We work with you to develop and approve a Care Plan.
