Seasons Adult Day Health Services is a therapeutic enrichment program held during the day in a group setting. It is for individuals living with a chronic illness, changes in memory or with a dementia such as Alzheimer’s disease. We specialize in socialization and programming, but we are fortunate to operate under a medical model. This means that we have a clinical Registered Nurse on staff full time. The nurse at Seasons performs routine nursing assessments, memory testing and depression screens. She can also administer medications and monitor for illness or conditions which could potentially end up in a hospital stay. Our goal is to keep participants out of the hospital and to stay as healthy as they can.
Our Seasons nurse, Susan Rosiek, is a skilled and competent dementia expert. She began her nursing career at St Luke’s in Saginaw, which later became Covenant. Susan worked there for over fifteen years, working medical- surgical, orthopedic-neuro and critical care units. Next, she worked at A&D Home Care and Region 9 doing Care Management and Home and community Based Waiver programs for over twenty years. Susan then transitioned to the PACE (Program of All Inclusive Care for the Elderly) program in Saginaw as a Home Care Coordinator. Before coming to Seasons, she worked at Community Village as their nurse for several years. She also has experience as a MMAP (Michigan Medicare/Medicaid Assistance Program) counselor and a facilitator for an Alzheimer’s support group.
