Senior Services provides the service of a Care Coordinator when an older adult in Midland County receives the services of Meals on Wheels, Homemaking, Personal Care, and a variety of other assistance services. There is no fee for the assessment and ongoing communications which are crafted to identify and provide guidance strategies, options, and resources to foster independence and enhanced quality of life. You’ll be reading stories of how lives are enhanced, barriers are removed, and burdens are lifted through care plans developed to meet needs. From the Care Coordination Team, we want to say it is a privilege to serve each of our clients, as every life matters and has significance in our community.
Since the beginning of the Coronavirus Stay At Home Order, the services of Care Coordination have also been under quarantine. For the safety of our clients and employees, services have been limited and home visits discontinued. All of the Care Coordinators, and many additional staff members, have maintained a commitment to call each client weekly to offer support. Many needs have been identified and met, and personal contact has been essential for morale and community camaraderie. We continue to accept new clients and offer assessments by telephone until we can meet face to face. Your Care Coordination Team would like to express appreciation for your patience as we make modifications to extend services within the safety guidelines during the pandemic.
Senior Services produced a beautiful time capsule journal and mailed it as a gift to our clients in May. The Care Coordination Team encourages you to use the journal, entitled Silver Linings, to record your thoughts to pass on to the next generation. We believe that your life, with all of its history, wisdom gained, and courage earned, is full of meaning and necessary for those just starting to live. If you did not receive a journal and would like to request one, please call 989-633-3700 or your Care Coordinator. We have a limited quantity on hand.
