Activity Coordinator at Seasons Adult Day Health Services
Located at: 4812 Dublin Ave. | Midland
(Full-time – 40 hours/week) – Primary responsibilities include developing group and individual activities for participants, training staff, monitoring facilitation, implementing group outings, completing assessments and volunteer oversight. Previous experience with older adults and hands-on skills working with people living with dementia is preferred. Must have patience and understanding, effective verbal and written communication and organization skills. Bachelors Degree in a Human Services field required. Bachelor’s Degree in Therapeutic Recreation and certification through the National Council for Therapeutic Recreation Certification preferred. A full benefit package is offered.

Care Coordinator
(Full-time – 40 hours/week) – Provide in-home assessment with frail and vulnerable older adults, develop a plan of care, coordinate necessary services and resources and provide on-going follow up and monitoring. The ability to work as a member of an interdisciplinary team is required. Experience working with family caregivers and older adults with physical and/or cognitive limitations preferred. Bachelor’s degree in a Human Service field preferred, or 2 years Case Management experience. Reliable transportation required to conduct home visits with clients in Midland County. Computer skills required. We offer mileage reimbursement (IRS mileage rate of $.70), competitive pay, benefit package and holiday pay. No nights, weekends or on-call. Direct Care Worker – Home Care
Part Time (20-29 hours per week) Provides personal assistance and support to older adults in their home. Assistance includes, but not limited to, bathing and grooming and basic household cleaning tasks. Physical ability to reach, bend, stoop, climb stairs and lift up to 25 lbs. required. Previous personal care experience and experience working with older adults preferred. Must have good communication skills, maintain client confidentiality and be dependable and able to get along well with older adults and their family caregivers. Ability to work compassionately with individuals with varying degrees of physical and cognitive limitations. Position requires driving within Midland County. Competitive pay, holiday pay and mileage reimbursement at the current IRS rate. High school diploma required, Certified Nurse Aide (CNA) not required. No nights, weekends or on-call.

On-call Cooks
Openings for On-Call Cooks in the Nutrition Department to provide kitchen assistance as needed at all Centers. This position has no guaranteed weekly hours or benefits, (called in when needed). The knowledge gained regarding food safety, preparation, service and clean-up will help to prepare you for potential future opportunities. Previous food service or commercial meal preparation experience preferred but not required.
